Making a Referral

Initial Enquiry

Send a referral form or care plan outline to youngconsultant11@outlook.com. We will confirm capacity within 24 hours.

Assessment

A senior manager will conduct a face-to-face assessment to understand risks, needs, and compatibility.

Proposal & Costing

We submit a detailed support proposal and transparent costing breakdown for panel approval.

Transition

Once agreed, we begin a structured transition plan to ensure a safe and settled start.
