We provide structured, person-centred support for young people and young adults navigating significant life transitions, recognising these periods as high risk for disengagement, emotional instability, and breakdown in support. Our work focuses on creating continuity, clarity, and confidence at times of change, ensuring individuals are supported before, during, and after transition points.

Understanding Transitional Risk

Transitions often involve changes in environment, expectations, relationships, and identity. Without appropriate support, individuals may experience anxiety, loss of routine, reduced engagement, or regression in progress. We understand that transitions are not single events but processes that require preparation, practical planning, and sustained follow-up.

Our approach is informed by observation of behaviour, emotional responses, and engagement patterns, allowing us to anticipate challenges and respond proactively.

School to College or Post-16 Provision

The transition from school to college, training, or alternative provision can be particularly challenging for young people with additional needs, low confidence, or previous negative educational experiences. Our support includes:

Custody to Community

Transitions from custody to the community present complex challenges, including loss of structure, exposure to previous risk factors, and difficulties accessing education, employment, or housing. We provide consistent, non-judgemental support focused on:
